  • This is a hybrid remote position. The selected candidate must reside in Washington State and be available to report to the office as required for meetings, trainings, or operational needs.

Job Summary

The Population Health Coordinator role is designed for individuals who possess a passion for and a commitment to helping underserved populations access to important preventative healthcare services while maintaining HIPAA compliance standards. The coordinator conducts outreach, participates in workgroups, and tracks productivity and program results.

Education

  • High school graduate or equivalent.

Preferred:

  • Associate degree or bachelor’s degree in public health.

Experience

  • Working in ambulatory care (2 years)
  • Performing outreach experience (2 years).
  • Working with HEDIS measures and value-based care quality improvement initiatives (1 year)
  • Working with Medicaid, Medicare, multi-ethnic and non-English speaking populations (1 year)
  • Working with healthcare information systems, such as electronic health record and practice management systems (1 year)

Preferred:

  • Experience in a multiple provider medical practice/ambulatory medical setting.

Credentials

  • Certified Medical Assistant in Washington State (obtained within three months of hire/appointment).

Job Specific Functions/Performance:

  • Conducts patient outreach by phone, e-mail and text to schedule appointments for services, tests and preventative screenings related to priority child health, women’s health and chronic condition management HEDIS metrics for patients enrolled in Medicaid, Medicare and Marketplace lines of business.
  • Reschedules missed appointments, identifies barriers to care by conducting Social Determinants of Health (SDOH), advocate for patients in need of services and refer as appropriate.
  • Participates in the development and implementation of outreach strategies and wellness campaigns. Works with managed care and community-based organizations. Track the outreach efforts.
  • Supports population health initiatives across multiple clinics, patient education, and engagement strategies to enhance participation in population health initiatives.
  • Conducts chart review of scheduled and missed appointments. Identifies open care gaps, clinical value review, and follow up from previous clinic visits.
  • Tracks and follows up on services completed.
  • Requests medical records to close care gaps, as needed.
  • Participates in Value Based Care (VBC) quality workgroups, improvement initiatives, workflow enhancements and the development of standard work.
  • Adheres to attendance standards in order to perform the job functions for daily operations and/or continuity of patient care.
